DARPA’s Defense Sciences Office convened Discover DSO Day, D3, on January 13, 2026 in Orlando to surface near-term opportunities and fast track partnerships that could reshape battlefield operations. The event was explicitly framed as an outreach and engagement forum where program managers described priority technical challenges in sensing, computation, energy, and resilience and where DARPA highlighted streamlined acquisition and short, disruption-focused efforts to move ideas from lab to field. Two themes stood out from the presentations and poster sessions that will matter most for combat casualty care. First, the convergence of AI, robust sensing, and autonomy is enabling unmanned ground vehicles to perform casualty evacuation and forward medical assessment tasks that were previously impossible without a human team nearby. Second, parallel investments in automated triage and remote physiological sensing are driving complementary capabilities designed to identify and prioritize wounded personnel without exposing additional medics to danger.

DARPA has been explicit in funding competitions and challenges to accelerate this convergence. The DARPA Triage Challenge, for example, funds multi-year efforts to pair aerial and ground robotic platforms with multimodal sensing and AI to detect and classify injuries at range. Teams in that program have used drones and ground robots equipped with thermal cameras, noncontact vital sign sensors, and machine learning classifiers to infer injury severity and recommend interventions. The competition structure and prize incentives are designed to push autonomy from operator-assisted modes to higher levels of decision support and, eventually, increased autonomy in detection and prioritization roles. At the hardware end, fielded unmanned ground vehicles are already configured for CASEVAC roles in active conflicts. Milrem Robotics’ THeMIS family has been sold, modified, and deployed for casualty evacuation, route clearance, and logistics in Ukraine, with multiple deliveries and operational feedback loops informing autonomy kits and payload integrations. These platforms illustrate the operational advantages - freeing personnel from high risk retrieval tasks and extending reach for medevac in contested terrain - but they also highlight a critical dual-use problem. THeMIS and similar platforms are modular by design and can be reconfigured for CASEVAC, resupply, route clearance, or kinetic payloads, which complicates legal, ethical, and doctrinal assessments when autonomy and AI are layered on top. Technically the pieces are approaching viability. Noncontact vital-sign sensing, thermal mapping, and automated wound classification have matured to the point where prototype systems can produce timely casualty location and severity estimates in structured exercises. DARPA-funded teams have demonstrated prototype pipelines that fuse sensor feeds, extract physiological signals, and output triage decisions scoring well on metrics like detection rate, classification accuracy, and time-to-assessment in test scenarios. The remaining hard engineering problems are mostly systems integration problems: ensuring reliable communications in degraded networks, maintaining power and thermal budgets for extended missions, making autonomy robust to adversarial sensing environments, and packaging medical algorithms to operate within constrained compute envelopes on UGVs. Where the D3 conversations became urgent was on the ethics and governance side. Three ethical fault lines deserve immediate attention.

1) Clinical accountability and the machine-made triage decision

Medical triage is inherently a value-laden clinical judgment. Moving part of that judgment to an AI system raises questions about clinical standards, liability, and acceptable error rates. Who is accountable when a false negative from an AI-enabled remote triage system delays care and a casualty decompensates? Program managers at D3 emphasized operational metrics and transfer pathways to field units, but the event format limited public-facing scrutiny and media participation, which reduces external oversight of clinical-risk assumptions. 2) Dual-use and mission creep

A vehicle optimized for CASEVAC but with an open architecture and modular payload bays creates a dual-use challenge. The same mobility, autonomy, and communications that make a UGV an effective CASEVAC also make it attractive for reconnaissance or weapons integrations. That modularity accelerates capability development, but it undermines trust when the same AI stacks are used for both life-saving and lethal configurations. Milrem’s own documentation acknowledges that THeMIS platforms can be rapidly reconfigured for multiple missions, which is a useful engineering choice but an awkward policy reality when autonomy is introduced. 3) Procedural transparency and battlefield information governance

Automated triage systems depend on clinical datasets, physiological priors, and labeled outcomes that are often developed in civilian medical contexts. Deploying those models in far-from-civilian environments risks distributional shifts and biased performance. Moreover, the data pipeline - from battlefield sensor capture to cloud aggregation - produces sensitive medical and geolocation data that merit strict governance. DARPA’s competitive, results-oriented approach drives rapid technical progress. It will take parallel investment in auditability, model documentation, and red-team evaluations to make field adoption responsible and legally defensible. Policy and technical guardrails are not the same thing, but both are required. From a practical perspective the community should prioritize four measures before widespread fielding:

  • Require human-in-the-loop or human-on-the-loop constraints for any system making life-and-death recommendations, with clear escalation logic and manual override capabilities.

  • Mandate provenance and audit logs for every triage decision, including sensor inputs, model version, confidence scores, and operator actions, to support after-action review and liability assessment.

  • Enforce strict modular-certification boundaries that differentiate CASEVAC configurations from lethal payload integrations, including physical demarcations and cryptographic configuration locks where appropriate.

  • Invest in robust, operationally representative validation datasets and red-team programs that simulate adversarial sensing, data corruption, and clinical edge cases before any transition to frontline units.

DARPA’s D3 event is doing what DARPA does well - lowering barriers between program managers and the research community and accelerating the maturation of high-risk, high-payoff ideas. The agency’s emphasis on sensing, computation, and disruption-style efforts will continue to push autonomous CASEVAC and triage technologies into tangible prototypes and field trials. Those outcomes are societally valuable. They also increase the urgency of a sober ethical and policy program that treats life-saving and lethal uses as inseparable from the start.

In short, the next wave of battlefield casualty-care technology looks less like a single gadget and more like a distributed system comprised of UGVs, drones, sensors, and AI models. That system can save lives and reduce risk to medics and soldiers, but only if we balance speed of innovation with enforceable governance, transparent validation, and clear lines of clinical and legal accountability.
